ROOTS Program

What is the ROOTS Program?

The School District of Oakfield is committed to promoting the health and wellness of all students. We are proud to offer a continuum of emotional and mental health support to meet our student's needs. 

In partnership with local agencies, the School District of Oakfield offers mental health services beyond what is offered in a typical school setting. These services are provided by a licensed therapist and are available during the school day.

Child Find

School districts are required to locate, identify and evaluate children with disabilities within their attendance area. Child Find is a continuous process of public awareness activities, screening and evaluation designed to locate, identify, and refer all children with disabilities who are in need of intervention as soon as possible.  The School District of Oakfield holds an annual Future Oaks Night for all children ages 2 ½ to 4 ½ in February or March.  Parents of young children with concerns about their development may also contact Holly Rabe, Director of Special Education / School Psychologist at 920-583-3146.

Annual Notice of Special Education Referral and Evaluation Procedures

Special Education Process

Upon request, the School District of Oakfield is required to evaluate a child for eligibility for special education services. A request for evaluation is known as a referral. When the district receives a referral, the district will appoint an Individualized Education Program (IEP) team to determine if the child has a disability, and if the child needs special education services. The district locates, identifies, and evaluates all children with disabilities who are enrolled by their parents in private (including religious) schools, elementary schools and secondary schools located in the school district.

A physician, nurse, psychologist, social worker, or administrator of a social agency who reasonably believes a child brought to him or her for services is a child with a disability has a legal duty to refer the child, including a homeless child, to the school district in which the child resides. Before referring the child, the person making the referral must inform the child's parent that the referral will be made.

Others, including parents, who reasonably believe a child is a child with a disability may also refer the child, including a homeless child, to the school district in which the child resides.

Referrals must be in writing and include the reason why the person believes the child is a child with a disability. A referral may be made by contacting Holly Rabe, Director of Special Education, School District of Oakfield, at 920-583-3146 or hrabe@oakfield.k12.wi.us.

504

Section 504 covers qualified students with disabilities who attend schools receiving Federal financial assistance. To be protected under Section 504, a student must be determined to: (1) have a physical or mental impairment that substantially limits one or more major life activities; or (2) have a record of such an impairment; or (3) be regarded as having such an impairment. Section 504 requires that school districts provide a free appropriate public education (FAPE) to qualified students in their jurisdictions who have a physical or mental impairment that substantially limits one or more major life activities. Students Experiencing Homelessness

The McKinney-Vento Act is a federal law that provides rights and services to children and youth experiencing homelessness.  If your child is experiencing homelessness, please complete this Residence Form and return it to the school office or contact Holly Rabe, homeless liaison for the School District of Oakfield, at 920-583-3146 or hrabe@oakfield.k12.wi.us.


Definition of Homelessness under the McKinney-Vento Homeless Assistance Act 

The McKinney-Vento Homeless Assistance Act defines homeless children and youths as those who lack a fixed, regular, and adequate nighttime residence. The law provides several examples of situations that meet the definition. This includes children and youths:

  • sharing housing due to a loss of housing, economic hardship, or a similar reason;

  • living in hotels, motels, trailer parks, or camping grounds due to a lack of alternative adequate housing;

  • living in emergency or transitional shelters;

  • abandoned in hospitals;

  • living in a public or private place not designated for, or normally used as, a regular sleeping accommodation for human beings;

  • living in cars, parks, public spaces, abandoned buildings, substandard housing, bus or train stations, or similar places; and/or

  • living in one of the above circumstances and who are migratory.
